New Patient Registration Form
We are accepting new patients and would be delighted to have you join our Medel Patient Family. If you fill in the registration form below an admin team member will contact you and confirm your details, this may take 1 week due to high volumes. You will then be booked in with a Doctor for a consultation to discuss your medical history and to ensure we can meet your medical needs *there will be the usual consultation fee for this appointment*
Medical Card patients are not accepted at this time but we do plan to change this policy before the end of 2022.
